Position Summary


This role is crucial in ensuring the safe and effective administration of medications. Responsibilities include verifying medication orders while assessing optimal drug selections, dosage regimens, and monitoring drug therapy services throughout the healthcare system.

The pharmacist will oversee both inpatient and outpatient medication therapies, collaborating with medical staff to identify suitable alternatives when necessary.

Additionally, the pharmacist will evaluate patient care plans and medication therapies, addressing any medication-related issues to optimize pharmaceutical care.


Position Responsibilities

  • Review and assess all medication orders for patients across all age demographics.
  • Maintain patient medication records within an automated pharmacy information system.
  • Provide consultation and guidance to medical, nursing, and other healthcare professionals regarding medication prescribing, dispensing, and administration.
  • Continuously monitor the patient's overall drug therapy for efficacy, side effects, adverse reactions, and potential drug interactions.
  • Conduct targeted drug monitoring programs and evaluations as required.
  • Document Pharmacist Intervention activities, including recommendations for drug selection and dosage adjustments.
  • Gather medication histories for selected patients and relay relevant information to healthcare providers.
  • Communicate effectively with physicians, nurses, and other healthcare professionals as needed.
  • Prepare and verify the correct preparation of medications for dispensing.

Qualifications

Requirements:

  • Registered Pharmacist with the Texas State Board of Pharmacy in good standing (RPH)
  • Completion of an ACPE approved 20-hour sterile products course demonstrating aseptic technique

Pay Range
$58.00 /hour
