Key Responsibilities

The Social Work and Admissions Specialist plays a crucial role in facilitating smooth transitions for clients from their initial admission through the clinical process and onward to subsequent care levels.

  • Engage with new patients and their families during the admission process, ensuring all necessary documentation is completed.
  • Act as a liaison between patients, families, and the care team to communicate needs effectively.
  • Collaborate with an interdisciplinary team to coordinate social services and plan transitions for each patient.
  • Ensure compliance with all relevant state and federal regulations while completing patient referrals.
  • Facilitate family communication as directed by the care team to support the delivery of recommendations.
  • Provide counseling and educational support to patients and families to address personal and social needs.
Qualifications

To be considered for this position, candidates should possess:

  • A Bachelor’s degree in a healthcare-related field is preferred.
  • In-depth knowledge of the social service requirements for the aging population.
  • A minimum of two years of experience in Social Work, Case Management, or a related area.
  • Experience in delivering social services, discharge planning, or managing care transitions in a post-acute or acute care environment.
