Key ResponsibilitiesThe Social Work and Admissions Specialist plays a crucial role in facilitating smooth transitions for clients from their initial admission through the clinical process and onward to subsequent care levels.
- Engage with new patients and their families during the admission process, ensuring all necessary documentation is completed.
- Act as a liaison between patients, families, and the care team to communicate needs effectively.

To be considered for this position, candidates should possess:
- A Bachelor’s degree in a healthcare-related field is preferred.
- In-depth knowledge of the social service requirements for the aging population.
- A minimum of two years of experience in Social Work, Case Management, or a related area.
- Experience in delivering social services, discharge planning, or managing care transitions in a post-acute or acute care environment.
