Senior Care Specialist

We are seeking a highly skilled Occupational Therapist to join our team in providing exceptional patient care. This per diem opportunity allows for flexibility and autonomy, working in a home setting to deliver effective treatment plans and support case management.
As an experienced Occupational Therapist, you will possess strong clinical skills and a passion for helping individuals achieve their full potential. Your expertise in acute care, occupational health, and patient service will enable you to conduct comprehensive assessments, develop tailored treatment plans, and collaborate with patients, families, and healthcare professionals to promote optimal health and well-being.
About the Role- Conduct comprehensive evaluations of patients' physical, cognitive, and emotional abilities to identify areas of strength and weakness.
- Develop and implement individualized treatment plans that incorporate speech therapy, physical therapy, and other modalities to improve patients' functional abilities and independence.
- Utilize medical terminology to accurately document patient progress, treatment plans, and outcomes.
- Collaborate with patients, families, and healthcare professionals to develop and implement treatment plans that address patients' unique needs and goals.
- Provide patient education and support to help patients and families navigate the healthcare system and manage their condition.
- Assist with case management by communicating with healthcare providers, social workers, and other stakeholders to ensure patients receive comprehensive care.
- A Master's or Doctoral degree in Occupational Therapy from an accredited institution.
- Current licensure as an Occupational Therapist in the state where the position is located.
- Experience working in a hospital, clinic, or other healthcare setting.
- Strong knowledge of medical terminology and ability to communicate effectively with patients and healthcare professionals.
- Experience with case management and patient service.
- Experience working with patients with a range of diagnoses, including those requiring critical care.

CompensationThis per diem opportunity offers a competitive salary of $60,000 - $80,000 annually, depending on experience and qualifications. We provide flexible scheduling and opportunities for professional growth and development.
