RN-Acute Care IRP Transformation Specialist

Responsibilities
- Provide clinical direction for all aspects of patient care, ensuring seamless coordination and communication among interdisciplinary teams.
- Foster a culture of patient safety, quality improvement, and evidence-based practices throughout the nursing process.
- Collaborate with multidisciplinary teams to identify and prioritize patient needs, developing individualized plans of care that promote effective outcomes.
- Stay up-to-date on regulatory agency requirements, including TJC, DHEC, and CMS standards, ensuring compliance and adherence to organizational policies.
- Participate in regional travel across Prisma Health sites, sharing best practices and expertise to enhance patient care.
Requirements
- Bachelor's degree in Nursing (BSN) preferred; Associate's degree in Nursing (ADN) or Diploma in Nursing accepted with two years of experience as a registered nurse.
- Licensed to practice as a Registered Nurse in South Carolina.
