Advanced Practice Healthcare Provider

We are seeking a highly skilled Nurse Practitioner to join our team at WellBe Senior Medical. As a key member of our care community, you will provide compassionate and high-quality medical care to our patients in the comfort of their own homes.
Key Responsibilities- Deliver primary, urgent, and acute medical care to patients with chronic and acute illnesses or injuries
- Maintain accurate and current patient records, and schedule initial, urgent, and follow-up appointments as required
- Work collaboratively with our physicians, social workers, paramedics, and care team coordinators to deliver concierge-level care to every patient
- Provide educational resources to patients and families to facilitate informed decision-making
- Perform initial patient screenings and physical examinations, create assessment plans, and address urgent care needs as needed
- Collaborate with community teams to complete care assessments, order and interpret diagnostic tests, and determine the need for follow-up appointments and further treatment options
- Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents
- Update patient records and check records for accuracy at each patient appointment
- Deliver quality care while maintaining all company metrics pertaining to documentation, coding, HEDIS measures, patient engagement, and medical expense
- Serve as a resource for patients in delivering ongoing care information, counseling, and guidance for critical conditions
- Refer to internal and external resources as appropriate, such as WellBe behavioral health protocols
- Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings
- Maintain required licensure and certification
- Deliver palliative care and end-of-life care to patients and families
- Facilitate family meetings with the WellBe community team to address complicated family dynamics and goals of care
- Master's Degree in Nursing
- Valid nurse practitioner license in the state of operations and current board certification in your field
- 5+ years of progressive care experience
- BLS and ACLS certification and an NPI# as an NP
- Valid driver's license, car insurance, access to a car, and willingness to drive to patient homes/patient locations
- Full-time permanent position with a flexible schedule and on-call rotation
