Job Title: Home Health LPN

At UMass Memorial Health, we are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team. As a Home Health LPN, you will provide high-quality patient care in the comfort of patients' homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities:
  • Provide patient care according to standards of practice and departmental policy and procedure.
  • Assist in patient education and document appropriately according to policy and procedure.
  • Achieve continuity of patient care through planning and exchange of information with other healthcare professionals.
  • Administer medications safely and in accordance with professional standards of practice and departmental policies and procedures within the scope of practice for the LPN.
  • Makes recommendations for changes to the plan of care to the appropriate RN or other team member as the patient condition warrants.
  • Delegate patient care activities to appropriate personnel in accordance with professional standards of practice and departmental policy and procedure and within the scope of practice.
  • Meet productivity guidelines (7 visits per 8-hour day) according to departmental policy and procedures.
  • Support performance improvement initiatives through active participation in PI processes, projects, and teams.
  • Create and maintain documentation, which is consistently complete, accurate, timely, and legible in accordance with departmental policies and procedures.
  • Maintain a safe environment for our patients through consistent compliance with policies and procedures.
  • Possess and apply the skills and knowledge necessary to provide care to patients and customers throughout the life span, with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.
Requirements:
  • Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts.
  • Successful completion of an accredited Licensed Practical Nurse Program.
  • Valid US-issued driver's license.
  • Registered, inspected, and insured automobile for work-related purposes.
Preferred Qualifications:
  • BLS certification.
  • CPR certification.
