SSM Health is seeking a skilled Registered Nurse to join our team as a Case Manager. As a key member of our healthcare team, you will play a vital role in ensuring patients receive the best possible care and support throughout their hospital stay.
Job Summary:The RN-Case Manager supports our organization by ensuring patients are discharged from the hospital safely, identifying any barriers and working through those with the patient and care team. This role requires strong communication and collaboration skills, as well as the ability to assess patients' needs and develop effective discharge plans.
Responsibilities:- Assesses patients at high risk for readmission and develops plans to prevent readmission
- Collaborates with the interdisciplinary team to develop and implement discharge plans
- Ensures compliance with Condition of Participation (CoP) discharge planning
- May be responsible for utilization management, ED case management, admission process, and liaison with post-acute providers
- Graduate of accredited school of nursing or education equivalency for licensing
- Three years' acute care experience
- Registered Professional Nurse (RN) licensure in Illinois, Missouri, or Oklahoma
