Job Summary

We are seeking a highly skilled Nurse Case Manager to join our team at the University of Oklahoma. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and case management services to our patients.

Key Responsibilities:
  • Provide clinical expertise and guidance to new and lower-level case managers, ensuring their success in case management.
  • Perform independently, requiring a broad understanding of principles and practices.
  • Assess critical need or high-risk patients to ensure appropriate care and participate in case management programs.
  • Advise caregivers, provide patient education, and make referrals for other services.
  • Collect in-depth information about clients' situations and develop comprehensive case management plans.
  • Monitor and evaluate treatment plans and progress, facilitating interdisciplinary approaches.
  • Develop and implement action-oriented case management plans, determining and documenting specific objectives and actions.
  • Organize and secure resources necessary to accomplish case management goals.
  • Measure and analyze interventions to determine outcomes and modify or change case management plans as needed.
Requirements:

Education: Bachelor's Degree in Nursing, with 12 months of professional nursing experience. Equivalent/substitution: Associate's degree in Nursing and 24 months of professional nursing experience.

Skills:
  • Knowledge of federal and state laws, particularly patient confidentiality.
  • Knowledge of case management fundamentals and technical/procedural aspects.
  • Knowledge of medical terminology.
  • Ability to resolve moderate to complex issues, applying judgment, critical thinking, and problem-solving skills.
  • High level of organizational skills to prioritize, organize, and complete projects or documents in a timely manner.
  • Ability to keep accurate, detailed records.
  • Ability to handle stressful situations and different circumstances.
  • Ability to set priorities and manage obstacles while maintaining focus on goals.
  • Ability to build and maintain professional relationships with clients, colleagues, and other agencies, following ethical standards of practice.
  • Self-motivated to achieve results, flexible when changes are necessary, and responsive to feedback in a positive manner.
  • Ability to analyze information and evaluate results to determine the proper course of action, following established standards.
  • Ability to develop and maintain cooperative working relationships.
Certifications:
  • State of Oklahoma Registered Nurse Certification.
Physical Requirements:
  • Physical: Sitting for long periods of time, speaking, and listening.
  • Environmental: Office Environment.
