About the Position

Lane County Behavioral Health is seeking a skilled Community Health Nurse to join our team. As a key member of our mental health services team, you will work collaboratively with our child, adolescent, and adult mental health services teams to provide support to individuals with severe and persistent mental illness.

Key Responsibilities:

  • Provide nurse case management and care management for individuals with severe and persistent mental illness, including those with co-morbid medical conditions.
  • Triage and service coordination with clinical and medical staff, primary care providers, and allied community agencies, including crisis intervention as required.
  • Health and wellness promotion through identification of mental health issues and intervention/prevention of health problems, skills training, and patient and family psycho-education.
  • Consult with primary care providers, psychiatry, emergency services, and other services on behalf of mutual patients.

About the Division

Lane County Behavioral Health provides services to over 1,600 individuals with behavioral health conditions. We work with community partners to provide a continuum of care from prevention through treatment and aftercare in an environment that promotes a culture of resilience and recovery.

Requirements

  • Bachelor of Science in Nursing degree from an accredited college or university.
  • Oregon Health Licensure required: Oregon Nurses License.

  • Oregon Driver's License.
  • Immunization records/TB within 10 days of employment.
