Nurse Practitioner or Physician Assistant

Key Responsibilities:- Conduct comprehensive assessments to identify patient needs and develop personalized care plans
- Manage patients through acute medical situations, including testing, diagnostics, and interventions
- Collaborate with interdisciplinary care teams to ensure accurate and complete documentation
- Communicate effectively with patients, families, and caregivers to provide education and support
- Facilitate agreement and implementation of patient plans of care, engaging facility staff, families, and primary care clinicians
