Renal Care Manager

Somatus is a leading value-based kidney care company that empowers patients with chronic kidney disease to live healthier lives. As a Renal Care Manager, you will play a critical role in improving patient outcomes by coordinating care across multiple settings.
Responsibilities- Conduct comprehensive assessments to identify patient needs and barriers to care.
- Develop and implement personalized care plans to address patient needs and improve health outcomes.
- Coordinate care across the continuum, including home, hospital, skilled nursing facility, and acute care settings.
- Provide education and support to patients and their families to promote self-management and shared decision-making.
- Collaborate with the care team, including nurses, dietitians, pharmacists, social workers, and physicians, to ensure seamless care coordination.
- 1+ years of nursing experience in case management or care management, preferably coordinating care across multiple settings.
- Current, unrestricted compact Registered Nurse license.
- Strong analytical and critical thinking skills, with the ability to work independently and as part of a team.
- Effective written and verbal communication skills, with the ability to interact with patients, families, and healthcare providers.
- Ability to travel throughout the assigned region and comfort with conducting home visits, as needed.
