We achieve this by integrating with oncology practices and health systems through the Collaborative Care Model (CoCM), ensuring our members receive a holistic approach to care.
Key Responsibilities- Conduct initial clinical member biopsychosocial intakes, including safety screens and administering behavioral health assessments.
- Engage in follow-up visits and asynchronous interactions, providing brief interventions and support.
- Coordinate care and serve as a liaison between referring specialists and the practice, ensuring timely responses to member questions and administrative tasks.
- Lead weekly interdisciplinary team meetings with the Consulting Psychiatrist and Health Coach, discussing high-risk members and documenting new psychiatric recommendations.
- Communicate closely with the Cerula Care behavioral health care team.
- Document member progress in a HIPAA-compliant electronic medical record system.
- Identify patients requiring more intensive mental health care and report concerns to the medical provider and Consulting Psychiatrist.
- Facilitate referrals for clinically indicated services outside of Cerula Care under the supervision of the BHCM Lead.
- Bachelor's Degree in nursing, social work, or a related health and human services discipline from an accredited college or university.
- Experience as a Certified Case Manager (CCM), Community Health Worker (CHW), or Peer Support Specialist (PSS) is preferred.
- At least one year of clinical care management experience, with some part of the experience directly working with people with cancer or advanced illness.
Preferably located within commuting distance of Roanoke or Blacksburg, VA. Part-time, hourly requirement (1099 contractor position) depending on caseload, with a starting hourly rate in the mid-20s.
