Provide compassionate and personalized support to individuals in their homes, empowering them to maintain independence and well-being.
Our Mission: At Caring Hearts of Georgia Health Services LLC, we are dedicated to enhancing the lives of our clients by delivering exceptional home care services. We strive to create a supportive and nurturing environment where individuals can thrive.
Your Responsibilities:
- Deliver personalized medical and personal care based on individual client needs, as outlined in their Plan of Care developed by our Nurse Supervisor.
- Facilitate transportation to appointments and social activities, ensuring safe and timely travel using your own vehicle or the client's vehicle.
- Communicate effectively and empathetically with clients, families, and our team members.
- Perform other duties as assigned to contribute to the overall well-being of our clients.
Qualifications:
- High School Diploma or GED required.
- Minimum one year of experience providing in-home care.
- Current Certified Nursing Assistant (CNA) certification or Personal Care Assistant (PCA) certification in the state of Georgia.
- Valid CPR and First Aid certifications.
- Tuberculosis (TB)/PPD test results.
- Driver's License or State ID.
Physical Demands: This role requires frequent to constant standing, walking, and hand use. You will also frequently twist, bend, stoop, kneel, crouch, and reach.
