About the Role

The Licensed Practical Nurse (LPN) will be responsible for delivering compassionate and high-quality care to patients, using their technical skills and clinical knowledge to make a positive impact on patients' lives. Your strong communication and interpersonal skills will enable you to build trust with patients and their families, ensuring they receive the best possible care.

Responsibilities

- Provide direct patient care, including administering medications, wound care, and other interventions as ordered by physicians
- Conduct thorough assessments and develop individualized care plans that address patients' physical, emotional, and social needs
- Collaborate with patients, families, and caregivers to educate them on disease management, self-care techniques, and safety precautions
- Maintain accurate records and documentation, including progress notes, treatment plans, and discharge summaries

Requirements

- Current LPN licensure in state of practice
- CPR certification required
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills
