Department: Personal Care Home (PCH)

Reports to: Personal Care Home Administrator

Position Objective:

The role is pivotal in delivering direct residential assistance to the PCH Administrator, Shift Coordinators, and Direct Support Professionals. Candidates must exhibit strong organizational capabilities and possess leadership potential. The ability to manage multiple tasks and handle high-pressure situations while upholding professionalism is crucial. Responsibilities include aiding in the provision of support and services for residents, scheduling staff to ensure compliance with ratios, and maintaining the health and safety of all residents.

Essential Qualifications:

A Bachelor's degree in Human Services, Business, or Social Services is preferred, alongside a minimum of 2 years of relevant work experience or a combination of education and experience totaling 4 years. Experience in the Human Services sector is desirable, with a demonstrated commitment to working with adults with developmental disabilities or intellectual disabilities.

Applicants must be at least 21 years old, possess a valid Class C Georgia Driver's License, provide proof of vehicle insurance, and comply with company policies regarding motor vehicle records. A GBI and FBI criminal history record check, including fingerprinting, is mandatory. Candidates must also pass a drug screening and tuberculosis screening.

Key Responsibilities:

  1. Availability for at least two evening, overnight, and weekend shifts monthly to support 24/7 operations.
  2. Ensure accurate narcotic medication counts in accordance with policy.
  3. Assist with medication administration as required.
  4. Familiarity with the Performance Development Protocol for medication errors and coaching Direct Support Professionals to minimize occurrences.
  5. Provide leadership in the absence of the PCH Administrator or Shift Coordinator.
  6. Manage staffing to meet regulatory ratios and staffing profiles, filling vacant shifts and covering call-outs.
  7. Work a schedule that maximizes availability to Shift Coordinators and Direct Support Professionals.
  8. Conduct and/or oversee daily residential shift updates.
  9. Monitor and assess the physical condition and maintenance of residential facilities using the Residential Inspection checklist.
  10. Be on-call during evenings, weekends, and holidays as scheduled.
  11. Act as the Designated Person of Authority when the PCH Administrator is unavailable.
  12. Ensure daily documentation is entered into Point Click Care by residential staff.
  13. Ensure timely and accurate completion of Incident Reports within the shift they occur.
  14. Exhibit excellent interpersonal skills and collaborate effectively with all levels of internal management and staff.
  15. Ensure all staff fulfill their assigned duties competently, providing quality service to residents, families, and colleagues.
  16. Possess advanced computer skills and the ability to train others in system usage.
  17. Oversee meals in the dining area and during special events as assigned.
  18. Monitor and assist community volunteers on campus.
  19. Plan schedules for newly hired residential staff, including PRN and part-time positions.
  20. Attend Individual Support Plan meetings as assigned.
  21. Perform additional duties as necessary to uphold the mission of Annandale at Suwanee, Inc. in providing personal service and care to residents.

Desired Personal Skills and Physical Requirements:

  • Proficient in reading, writing, and speaking English.
  • Adept at problem-solving and managing a variety of concrete variables in situations with limited standardization.
  • Ability to interpret diverse instructions presented in written, oral, diagrammatic, or schedule formats.
  • Regularly required to use hands, fingers, and arms; talk and hear.
  • Frequently required to sit, stand, walk, climb, balance, stoop, kneel, crouch, bend, or crawl.
  • Must be able to lift up to 25 pounds and/or move up to 75 pounds with assistance.
  • Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Must be able to transport (drive) and assist residents in and out of vehicles.
  • Must be capable of performing CPR and First Aid.
