Job Description

Alternate Solutions Health Network LLC is seeking a skilled Speech Language Pathologist to join our team in Cuyahoga Falls, Ohio.

Job Summary

We are looking for a compassionate and experienced Speech Therapist to provide home-based skilled speech therapy care for patients. As a Speech Language Pathologist, you will educate patients and their family members, ensure patient safety, and make adjustments to the treatment plan as needed.

Responsibilities
  • Develop an individualized ST plan of care and make adjustments as needs change.
  • Guide the patient in his/her use of therapeutic, creative, and self-care activities to improve functioning.
  • Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
  • Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication, and/or any other speech therapy-related function.
  • Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.
  • Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy.
Requirements
  • Speech Therapist with a current license in the state of employment.
  • Minimum one-year experience as a ST in an acute care setting. Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Capable of all physical demands.
