About the Role:As an LVN, you will play a vital role in providing high-quality patient care to individuals with brain, spinal cord, and medically complex injuries and illnesses. You will work in a residential setting, providing sub-acute physical rehabilitation and supporting patients in achieving their goals.
Responsibilities:- Conduct assessments and implement healthcare plans to ensure patient needs are met
- Communicate effectively with physicians, external medical centers, and team members to ensure consistency of medical information
- Monitor and ensure physician orders are documented and carried out as prescribed
- Provide patient education and support to promote independence and well-being
- Current state LPN/LVN license
- Current CPR certification
- Current physical dated within the past year
- Excellent assessment skills and critical thinking in the clinical environment
- Effective communication skills and ability to work with diverse populations
