Job Summary
We are seeking an experienced Home Care Registered Nurse (RN) to join our team. As a Home Care Registered Nurse, you will play a vital role in providing professional nursing services to our patients in their homes. Your expertise and compassion will make a significant difference in the lives of our patients and their families.
Responsibilities
- Interdisciplinary Care Planning
Actively participate in the development and implementation of patient care plans in collaboration with our interdisciplinary team.
Professional Nursing ServicesProvide high-quality nursing care to patients in their homes, adhering to established standards of practice and regulatory requirements.
Patient Education and SupportEmpower patients and their families with the knowledge and skills necessary to manage their health conditions and promote independence.
Documentation and CommunicationAccurately and thoroughly document patient information, communicate effectively with the healthcare team, and maintain confidentiality of patient records.
Continuous Quality ImprovementParticipate in quality improvement initiatives to enhance patient outcomes, safety, and satisfaction.
Requirements
- Nursing Education and Licensure
Successful completion of a nursing education program from an accredited institution and current RN licensure in the state where the branch is located.
Clinical ExperienceAt least one year of clinical nursing experience in an acute care or long-term setting, with home health experience preferred but not required.
Knowledge and SkillsCurrent knowledge of fundamental medical-surgical nursing principles and practice, excellent clinical nursing skills, and ability to communicate effectively, both verbally and in writing.
Certifications and TrainingCPR certification and ability to use a computer and have a working knowledge of various computer applications.
