Job Summary

We are seeking a compassionate and skilled Home Health Lead Caregiver to join our team at Stay Graceful. As a Home Health Lead Caregiver, you will provide direct supervision and daily care to our clients in their home environment, ensuring their physical and emotional well-being.

Responsibilities
  • Implement and maintain a professional and caring relationship with clients, providing reliable care, limit setting, and boundaries while demonstrating understanding of their needs.
  • Plan and participate in activities with clients as outlined in the care plan, promoting their independence and quality of life.
  • Attend and actively participate in weekly team meetings, group supervision, and complete in-service hours annually as required by state regulations.
  • Maintain a professional, courteous, and helpful relationship with clients' family and other primary social supporters.
  • Adhere to Stay Graceful's Standard Operations Procedures and South Dakota State Regulations/Administrative Rules relating to in-home care.
  • Assure all clients, coworkers, and family members are treated equally and with respect.
Requirements
  • No degree required; applicable experience in Human Services or Home Health experience is strongly preferred.
  • Excellent communication skills, including email, Word, and medical records.
  • Current South Dakota driver's license and proof of insurance.
  • Cleared through the Division of Criminal Investigation.

Work Location: Hybrid remote in Sioux Falls, SD 57106

Job Type: Full-time

Expected hours: 30 – 40 per week

Pay: From $17.00 per hour
