Job Title: Home Health Case Manager

Falcon Healthcare is a leading provider of home health services, with a strong presence in Texas and New Mexico. We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Case Manager.

Job Summary:

The Home Health Case Manager will be responsible for ensuring quality patient care and services are provided to in-home clients. This role requires strong organizational and communication skills, as well as the ability to make decisions based on educational preparation and experience in nursing.

Key Responsibilities:
  • Assure quality patient care and services are provided to in-home clients
  • Make decisions based on educational preparation and experience in nursing
  • Develop and implement individualized care plans for patients
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Provide education and support to patients and their families
Requirements:
  • Current RN License - active and in good standing
  • Minimum of 1-year nursing experience preferred
  • Home Health experience is a plus
  • Current CPR certification
  • Strong organizational and communication skills
  • Proof of current auto insurance
  • Proof of Covid vaccine, medical exemption or religious exemption
