Position Summary
As a Rehabilitation Specialist, you will assess, design, and implement comprehensive Physical Therapy programs based on physician referrals. Your primary goal will be to restore function, prevent disabilities, and assist patients in achieving their highest level of independence. You will also engage in supervisory responsibilities and ongoing professional development.Key Responsibilities
- Conduct thorough evaluations of patients to develop tailored care plans that incorporate necessary modalities, procedures, exercise regimens, and educational resources based on individual diagnoses and conditions.
- Collect and analyze relevant data from medical records, physician notes, and patient/family interactions to determine the need for assistive and orthotic devices.
- Maintain accurate documentation of evaluations, treatment progress, daily notes, attendance, patient education, outcomes, discharge summaries, and billing records.
- Reassess patients as necessary, adjusting treatment goals and plans in response to changes in patient status.
- Effectively communicate patient evaluations and progress to physicians, referral sources, and other healthcare professionals, coordinating treatment schedules and discharge planning while addressing patient care challenges.
- Provide and document educational resources for patients and families regarding treatment progress, therapeutic techniques, and personalized home exercise programs, including equipment usage.
- Oversee and mentor Physical Therapist Assistants, Physical Therapy Technicians, students, and volunteers, contributing to the orientation of new staff members.
Adequate hearing (with correction) for effective communication. Vision (with correction) sufficient for reading. Clear speech and cognitive skills necessary for job functions. The role requires frequent sitting, standing, and walking throughout the day, with occasional climbing, kneeling, twisting, and bending. Lifting of patients, equipment, or supplies may be necessary, with frequent lifting up to 20 pounds and occasional lifting up to 50 pounds. Rarely, lateral transfers of patients weighing up to 100 pounds may be required. You must be capable of demonstrating exercises and activities to patients and caregivers. Personal Protective Equipment such as gloves, goggles, gowns, and masks may be necessary due to potential exposure to hazardous materials. The work environment is dynamic and primarily indoors, with occasional outdoor responsibilities.
Qualifications
A Bachelor's Degree in Physical Therapy from an accredited institution is required. A valid NC Physical Therapy license is mandatory. CPR Certification and Basic Life Support for Healthcare Providers (BLS HCP) from the American Heart Association are required.-
