The Registered Nurse Manager is tasked with overseeing and orchestrating the inpatient care transitions program. This role is crucial in ensuring that patients navigate through the continuum of care effectively, achieving optimal clinical and financial results. The manager will actively participate in the development and execution of care transition integration across various care settings.
Collaboration is key; the manager will work closely with a multidisciplinary team, engaging with ambulatory services, post-acute providers, and health plans. The role also involves mentoring staff by exemplifying professional practices that align with organizational objectives, customer service excellence, clinical effectiveness, and care standards.
Key Responsibilities:
- Oversee departmental budget and ensure compliance with federal, state, and regulatory standards.
- Provide leadership and guidance to the case management team.
- Facilitate effective patient care transitions and enhance patient outcomes.
Qualifications:
- A minimum of 1 year of leadership experience.
- At least 3 years of experience in Acute Care Case Management.
- Bachelor's degree in nursing; Master's degree preferred.
- Current Registered Nurse Licensure.
- Specialty certification required within one year of eligibility (e.g., ACM, CCM, CCCTM, CMAC, or CMGT-BC).
- BLS certification within 90 days of hire.
