Job Responsibilities

  • Consult with clients to determine needs, coordinate services, and collaborate with the care team to evaluate interventions.
  • Provide comprehensive care, developing, monitoring, and evaluating each case to ensure optimal outcomes.
  • Identify clients requiring higher levels of care and link them to other services, including psychiatry, partial hospitalization programs, intensive outpatient psychotherapy, and ketamine-assisted psychotherapy.
  • Communicate with clients and primary clinicians to discuss possible care plans and linkage to other services.
  • Ensure quality scheduling and clinician schedules, receiving and admitting clients while coordinating care.
  • Coordinate admissions and scheduling for higher-level programs, utilizing electronic medical records to document and track case management services and referrals.
  • Foster client understanding and respect while maintaining confidentiality, collaborating with clinicians and providers to set up appointments.
  • Follow up with clients after 2-3 days of admission to assess treatment progress and provide support.
  • Maintain client rights by respecting confidentiality and adhering to organizational guidelines and healthcare laws.
  • Develop interdisciplinary management tools by participating in weekly meetings with supervisors, coordinating information, and care requirements.
  • Promote different mental health services, providing exceptional customer service and producing high-conversion rates.

Salary Information

The estimated annual salary range for this position is $52,000-$54,400, depending on related experience and qualifications.

Requirements

  • Bachelor's degree in Social Work or a related field.
  • At least 3 years of experience in a medical setting.
  • Prior psychiatric/mental health experience is desirable.
  • Customer service and sales experience are essential.
  • This role requires frequent travel to multiple facilities.

  • Excellent communication and interpersonal skills.
  • Computer proficiency in MS Office, Google Docs, and Gmail.
  • Strong analytical thinking and ability to multitask.
  • Knowledge of healthcare practices and procedures.
  • Ability to work effectively with diverse populations and maintain confidentiality.
