About the Job:

The Case Manager Registered Nurse will be responsible for managing patient care, developing treatment plans, and coordinating with healthcare providers to ensure seamless delivery of services.

Key Responsibilities:
  • Manage patient care and develop individualized treatment plans
  • Collaborate with healthcare providers to ensure comprehensive care
  • Coordinate with other departments to optimize patient outcomes
Requirements:
  • Bachelor's degree in Nursing or related field
  • Current RN licensure in ME
  • Minimum 1 year of experience as a Case Manager or similar role
