Job Summary:

As an Outpatient Coder Specialist at Kaiser Permanente, you will be responsible for accurately assigning diagnosis and procedure codes to patient health information records. This role requires a strong understanding of coding classification schemes, including ICD-10CM, HCPCS/CPT, and the ability to communicate with physicians to clarify diagnoses and procedures.

This position involves reviewing patient health information records to identify and assign appropriate codes, as well as serving as a consultant to care providers. You will also be responsible for identifying discrepancies, potential quality of care, and billing issues, and recommending and facilitating plans of action to correct these issues.

Essential Responsibilities:

Uphold and maintain Kaiser Permanente policies and procedures, as well as applicable state, federal, and local laws.

Review patient health information records to identify and assign appropriate codes for diagnoses, procedures, and other services rendered.

Appropriately assign and sequence codes for diagnoses, procedures, and other services as needed for proper Ambulatory Payment Classification (APC) assignment.

Serve as a consultant to care providers and interact with physicians through established query processes to clarify documentation supporting accurate patient diagnostic and procedure coding.

Abstract patient information into computerized systems, ensuring accuracy and integrity of data.

Ensure timely coded record availability according to regulatory guidelines, meeting established coding and abstracting productivity standards.

Ensure quality standards by meeting established 95% coding accuracy and 98% completeness quality standards.

Maintain and comply with HIPAA policies and procedures for privacy and confidentiality of all patient records.

Attend and participate in selected national, regional, and coding educational sessions.

Work collaboratively with others on coding questions and issues.

Demonstrate knowledge of system security, complying with KP Electronic Assets Usage Policy.

Maintain courteous and cooperative relations when interacting with others.

Perform other duties as assigned.
