Responsibilities- Conduct comprehensive assessments of patients' needs, including OASIS assessments, to develop personalized treatment plans.
- Collaborate with physicians and other healthcare professionals to revise treatment plans as needed.
- Teach, supervise, and counsel patients and their families on physical therapy programs.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Document patient progress and outcomes in a timely and accurate manner.
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current BLS Certification.
- Valid driver's license and reliable transportation.
- At least one year of experience as a Physical Therapist, with home healthcare experience preferred.
- A supportive and collaborative work environment.
