Job Overview
As a vital member of the Prisma Health Patient Flow Operations team, you will engage in essential activities related to Central Bed Management and the Transfer Center.
Central Bed Management plays a crucial role in the allocation of inpatient and observation patients, ensuring that admission, discharge, and transfer (ADT) orders are meticulously reviewed alongside bed requests, in collaboration with physician evaluations.
The Transfer Center is dedicated to facilitating direct admissions and patient transfers from Prisma Health facilities as well as from external healthcare institutions and physician practices.
Moreover, both Central Bed Management and the Transfer Center are instrumental in coordinating patient admissions, transfers, and placements, aligning with the capacity and capabilities of each facility or unit.
- Standardize processes to guarantee that patients are admitted to the correct unit with the appropriate orders.
- Implement standardized procedures for inpatient transfers and patient discharges.
- Assess bed turnaround times and formulate action plans to address identified opportunities.
- Enhance patient throughput and ensure safe, timely transitions of care for our patients.
- Optimize the utilization of existing systems to support patient flow.
- Collaborate with the Emergency Department, inpatient units, Administrators on Duty, and Environmental Services to enhance care transitions.
- Perform additional duties as assigned.
Supervisory/Management Role
This position does not involve management responsibilities and will report to a supervisor, manager, director, or executive.
Qualifications
- Graduate of an accredited nursing program recognized by the SC State Board of Nursing is required. A Bachelor's degree in Nursing is preferred. Work experience cannot substitute for the educational requirement.
- A minimum of three (3) years of experience in acute care nursing is required.
