The Healthcare Outreach Liaison plays a pivotal role at Upward Health. As the first point of contact for our patients, this individual must make an excellent impression and effectively communicate our service offering. The goal is to inform patients about our services and help them get started on their journey to improved health.

This role manages a caseload and utilizes various strategies to connect with potential patients. Phone outreach is our top strategy, and the ideal candidate should be comfortable and confident communicating by phone. There may be instances where field-based approaches are used as well. The Healthcare Outreach Liaison reports directly to our Outreach Manager.

We're seeking individuals who excel at building relationships with people from diverse backgrounds. You'll enjoy talking to new people, whether by phone or in person, and have a passion for helping others. Past experience in sales, healthcare, or outreach roles is highly valued. Effective communication, adaptability, and a drive to reach goals are essential qualities for success in this position.

Key Responsibilities:

  • Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies
  • Engage patients in conversations about the role that Upward Health can play in improving their health
  • Enroll patients into our program and collect key data during the enrollment process
  • Manage a personal caseload and ensure each assigned patient receives effective outreach
  • Interact with enrolled patients to maintain engagement or deliver additional community-based touchpoints as needed
  • Participate in weekly team meetings focused on ongoing education and improvement
  • Accurately and timely document interactions in our company's software system

  • Minimum of 3 years of work experience, preferably in sales, healthcare, or outreach roles
