We're seeking a highly motivated Patient Care Advocate to join our team. As a Patient Care Advocate, you'll play a vital role in delivering chronic care management to high complexity patients.

Your primary responsibility will be to conduct direct outreach to patients via phone, in-person, mailings, and other strategies to enroll them into our program and collect key data about their needs.

You'll also meet patients in their homes or in the community to conduct needs assessments, set health goals, and coordinate care between patients and interdisciplinary care teams as needed.

Key Responsibilities:

  • Coordinate care between patients and interdisciplinary care teams as needed.
  • Support patients in meeting their healthcare goals related to quality measures.
  • Analyze patient data to determine patient needs or treatment goals.

Requirements:

  • High school graduate or GED required.
  • A valid driver's license and auto liability insurance.
  • Reliable transportation and ability to travel within assigned territory or other locations as needed.
  • Experience in chronic care management or working with chronically ill/elderly patients preferred.
  • Ability to complete Upward Health's initial training program and ongoing educational requirements as assigned.
  • Basic computer skills, including typing.

The estimated salary for this position is $45,000 - $50,000 per year, depending on experience.
