Nurse Care Specialist

We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health RN. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes. Our team is dedicated to delivering exceptional care and support to our patients and their families.
Responsibilities
Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
Conduct thorough assessments of patients' physical and emotional needs, developing personalized care plans to meet their unique requirements
Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination
Educate patients and their families on disease management, medication adherence, and healthy lifestyle habits
Document patient progress, including treatment plans, medication lists, and vital signs, in a timely and accurate manner
Requirements
Current RN licensure in the applicable state
One year of experience in home health or a related field
Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
Ability to work independently, with minimal supervision, and prioritize tasks to meet deadlines
