Job Summary

We are seeking a highly skilled and experienced Health Home Operations Coordinator to join our team at Driscoll Children's Hospital. The successful candidate will be responsible for collaborating with various departments to assess, plan, develop, and implement operational activities related to the Health Home Model.

Key Responsibilities
  • Collaborate with internal and external stakeholders to track progress and impact of Health Home initiatives.
  • Forecast, monitor, analyze, and compile data to inform proactive strategies and recommendations.
  • Participate in clinical and operational processes to improve provider and member services and outcomes.
  • Develop and implement data tracking and monitoring systems to optimize Health Home program effectiveness.
  • Perform internal audits and case assessments to identify opportunities for improvement.
  • Facilitate evaluation of Health Home program effectiveness and provide training materials for internal and external stakeholders.
  • Communicate with network physicians and other stakeholders to promote Health Home initiatives.
Requirements
  • Bachelor's degree in Nursing, Population Health, or related field required; Master's degree preferred.
  • At least two years of relevant project management and/or performance improvement experience preferred.
  • Previous experience in managed care and healthcare preferred.
  • At least three years of healthcare experience required.
  • Experience with computer programs, including Microsoft Office, required.
Preferred Qualifications
  • Current unrestricted Registered Nurse Licensure in the state of Texas (if applicable).
  • CPHQ certification preferred.
  • Clinical Informatics certification desirable.
  • Project Management certification desirable.
  • Prior experience working with clinical information systems preferred.
