Job Summary:

The Customer Care Center (C3) Intake Coordinator is responsible for facilitating the transition of patients to home-based services. This role involves communicating with patients, families, and healthcare professionals to ensure seamless care coordination. Key responsibilities include determining appropriateness of care, gathering medical and insurance data, and documenting in the electronic medical record (EMR).

Key Responsibilities:
  • Determine appropriateness of care and referral screening based on THAH guidelines.
  • Gather and verify insurance and financial data from multiple sources.
  • Accurately document in the EMR and manage referrals from start to finish.
  • Coordinate and document care types, order details, and required information for safe patient transition.
  • Ensure referrals are managed with high customer service standards, verify and communicate accurate billing and demographic information, and document correct information in the EMR for billing purposes.
Requirements:
  • Associate degree preferred; High School diploma required with equivalent 2-4 years industry experience.
  • 2-3 years home care, hospice, discharge planning, or case management experience preferred.
  • Knowledge of third-party/managed care contracts related to THAH services preferred.
  • Working knowledge of standard office equipment and remote technology required.
  • Previous experience working in a healthcare environment, demonstrating computer literacy.
