Adecco Healthcare & Life Sciences is seeking a skilled LPN Case Manager to join their team in Kissimmee, FL. This is an in-person position with a contract possibility of extension or permanent offer.

Job Summary:

The LPN Case Manager will be responsible for discharge planning, liaison between patient, family, and provider, arranging referrals as needed, evaluating ongoing patient recovery after discharge, and accurately updating patient records.

Requirements:

  • HS Diploma or GED
  • Graduation from an accredited LPN program/Associate degree
  • Florida State LPN licensure – active and in good standing
  • Chart review experience
  • Outstanding time management skills
  • Great communication skills
  • Strong documentation skills
