We are seeking a highly skilled Licensed Clinical Social Worker to join our team at Horizon Health. As a Licensed Clinical Social Worker, you will be responsible for providing high-quality psychotherapy services to patients in assessments, diagnoses, treatment planning, group treatment, individual treatment, family treatment, and discharge planning.
Responsibilities:- Design and implement a treatment plan of care for patients based on a thorough assessment of the patient's symptom presentation, professional/referral input, and all other available data.
- Participate in referral development processes and be an active member of the referral development team.
- Assess patient progress and communicate results and trends to supervisor and treatment team.
- Conduct individual and/or psychotherapy sessions in accordance with state/regulatory standards, displaying sufficient theoretical knowledge of psychopathology and effective treatment techniques.
- Provide education, resources, and support for patients before, during, and after treatment.
- Engage in timely, efficient discharge planning, working closely with patient family, nursing staff, and attending physician.
- Complete progress notes and assessments according to program procedures.
- Measure patient performance and communicate results and trends to supervisor.
- Participate as an active member of the interdisciplinary and treatment teams.
- Master's Degree
- Minimum five years' experience
- Valid state license
- CSWE accreditation
- Crisis prevention experience
- Ability to manage anger, fear, hostility, and violence of others
- Ability to manage stress appropriately
- Proficient in applicable software applications
- Excellent interpersonal skills
- Communicates effectively with other professionals and support staff to achieve positive patient outcomes
- Above-average organizational skills
