About the Role:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in the Twin Cities Metro area. As an LPN, you will have the opportunity to work independently in a home care setting, providing one-on-one care to patients and making a meaningful difference in their lives.

Responsibilities:
  • Participate in ongoing interdisciplinary assessment of the patient
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide services that are ordered by the physician as indicated in the plan of care
  • Recognize and report changes in the patient's condition to the RN
  • Counsel and educate the patient and family in meeting healthcare-related needs
  • Complete timely and accurate clinical notes, including addressing patient progress
Requirements:
  • Current license as an LPN in the State of MN
  • 2 years' experience as an LPN
  • Able to travel within the geographic area serviced by the office
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • CPR Certification
