Key Responsibilities- Evaluate patients based on departmental standards and develop individualized treatment plans to achieve optimal outcomes.
- Conduct thorough assessments and modify treatment plans as needed to address patients' symptoms, activity levels, and medical status.
- Administer physical agents and manual therapeutic techniques to relieve pain and promote functional activity.
- Educate patients and families on home exercise programs and functional activities to enhance independent living.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Accurately document patient charges and adhere to departmental policies and procedures.
- Participate in continuing education and professional development to stay current with best practices and advance treatment techniques.
- Graduate of an accredited Physical Therapy program with a minimum of a B.S.
- Current Connecticut license in Physical Therapy.
- Strong English communication skills, both verbal and written.
- Personal computer skills and proficiency in Windows-based programs.
- Reliable transportation to travel between multiple work sites.
