Healthcare Coordinator Specialist

We are seeking a highly skilled Case Management Registered Nurse (RN) to join our team at Wellspring Nurse Source. As a case manager, you will play a vital role in coordinating and managing the care of patients, ensuring they receive appropriate, cost-effective, and timely healthcare services.
Key Responsibilities:- Conduct Comprehensive Assessments: Conduct thorough assessments of patients' medical, psychosocial, and financial needs to develop individualized care plans.
- Care Coordination: Coordinate and facilitate care across multiple healthcare settings, ensuring continuity of care, appropriate utilization of resources, and adherence to treatment plans.
- Collaboration with Healthcare Providers: Collaborate with physicians, healthcare providers, and insurance companies to authorize treatments, secure necessary services, and ensure compliance with clinical guidelines.
- Patient Education: Educate patients, families, and caregivers on disease management, treatment options, and available resources to support recovery and self-care.
- Advocacy: Advocate for patients to ensure they receive appropriate services and that care is delivered efficiently and effectively.
This is a full-time position offering an estimated salary of $80,000 - $100,000 per year, based on 8 hour shifts and 40 hours per week. The ideal candidate will have a Bachelor of Science in Nursing (BSN) degree, valid Registered Nurse (RN) licensure, and prior experience in case management, care coordination, or clinical nursing.
