Position Overview:
We are looking for a skilled and compassionate Speech Language Therapist (SLT) to become a vital member of our multidisciplinary rehabilitation team. The ideal candidate will possess formal education and hands-on experience in delivering feeding therapy services.
In partnership with patients and their families, the SLT will ensure the safe and effective provision of care within their professional scope. This role encompasses assessment, evaluation, diagnosis, treatment planning, intervention, and outcome measurement.
Work Schedule:
This position is part-time, with the potential to work up to 80 hours per pay period.
The SLT will provide services across various locations, enhancing patient care in multiple settings.

Key Responsibilities:
The Speech Language Therapist collaborates with patients and families during the evaluation and re-evaluation process.
1. Collects relevant data
2. Conducts thorough assessments
3. Establishes treatment diagnoses
4. Develops personalized care plans in partnership with patients and families
5. Analyzes information to identify needs and potential risks
6. Determines recovery prognosis
7. Sets individualized goals with patients and/or caregivers
8. Tailors care plans to accommodate developmental and cultural considerations
9. Implements and adjusts care plans based on ongoing assessments and patient feedback
10. Coordinates care and communicates effectively with the interdisciplinary team
11. Develops appropriate discharge plans
12. Provides skilled interventions in partnership with patients and families
13. Implements care plans
14. Modifies treatment plans as necessary based on patient progress
15. Assesses educational needs of patients and caregivers, providing relevant instruction
Documentation and Compliance:
1. Utilizes appropriate medical terminology
2. Adheres to professional practice standards
3. Completes documentation promptly according to departmental guidelines
4. Follows billing standards set by the department
Professional Expectations:

Qualifications:
Required:
1. Certification from the American Speech-Language-Hearing Association (ASHA) and a valid Speech Language Pathology license in the state of Minnesota, unless in Clinical Fellowship Year.
2. Active Basic Life Support certification in Minnesota.
Preferred:
1. Completion of Clinical Fellowship Year.
2. At least one year of relevant experience.
