Job Title: LPN AM Shift

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Artesia Healthcare & Rehabilitation Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Job Summary:

The LPN will be responsible for assessing patients, administering medications, participating in care planning, and communicating with physicians and other healthcare professionals. You will work collaboratively with our interdisciplinary team to ensure that our residents receive the best possible care.

Responsibilities:
  • Conduct thorough patient assessments, including physical examinations and diagnostic testing, to determine health status.
  • Administer medications and treatments as prescribed by physicians.
  • Participate in the care planning process to develop and implement individualized care plans.
  • Communicate effectively with physicians, nurses, and other healthcare professionals to ensure seamless care coordination.
  • Supervise and mentor LPNs and certified nursing assistants to ensure high-quality patient care.
  • Document patient assessments and care in compliance with standards of care and company policy.
  • Educate patients and their families on health-related issues and provide emotional support.
Requirements:
  • Graduate of an accredited school of nursing with a valid LPN license.
  • Current CPR certification.
