Key Responsibilities:
1. Provide rehabilitation therapy services to patients in a SNF setting.
2. Collaborate with healthcare team to develop and implement individualized treatment plans.
3. Conduct assessments and evaluations to determine patient needs and goals.
4. Develop and implement treatment plans to improve patient function and independence.
5. Communicate effectively with patients, families, and healthcare team to ensure quality care.

Requirements:
1. Current licensure as an Occupational Therapist in the state of NY.
2. Ability to work in a fast-paced SNF environment.
3. Strong communication and interpersonal skills.
4. Ability to work independently and as part of a team.
