Department Overview:

The Practice Optimization Program (POP) serves as a vital resource, ensuring that healthcare providers and their teams are engaged and empowered to thrive within a patient-centered, clinically driven value-based care framework. This initiative strengthens the physician-patient relationship, fostering sustained engagement to address patient needs effectively, ultimately impacting costs, quality, access, and overall patient satisfaction.

POP focuses on the priorities of providers and their teams, enhancing workflows and processes to improve the care experience for both patients and providers.

Key Responsibilities:
  • Establish and maintain ongoing engagement within practices to enhance costs, quality, access, and patient experience through strategic methodologies.
  • Develop and nurture strong relationships with practice leadership to inform performance improvement initiatives.
  • Serve as a subject matter expert in value-based care, proactively identifying educational needs and addressing barriers within the practice team.
  • Create and implement tailored practice optimization strategies based on data analysis, including measurable objectives as part of a rapid cycle optimization approach.
  • Conduct regular optimization meetings with practice leadership to monitor progress and identify opportunities for improvement.
  • Utilize data to analyze trends and collaborate with stakeholders to implement proactive strategies that enhance the impact of the Practice Optimization Program.
  • Support the improvement of physician satisfaction, addressing personal and professional well-being to mitigate burnout.
  • Assist the Enterprise Practice Optimization Team and leadership in summarizing and measuring the impact of the Practice Optimization Program.
A Day in the Role:
  • Engage independently within assigned practices, conducting regular assessments of healthcare data.
  • Implement rapid cycle optimization to ensure quality patient care delivery and monitor outcomes.
  • Collaborate with internal partners to support deployed optimization strategies.
  • Maximize the use of technology to enhance practice efficiency.
Success Metrics:
  • Achieve quarterly performance indicators related to engagement and measurable outcomes.
  • Maintain a comprehensive understanding of client measures and requirements.
  • Identify and collaboratively resolve issues affecting practice performance.
  • Ensure stakeholder buy-in for changes implemented within practices.
Qualifications:
  • Bachelor's degree in Business or Healthcare or equivalent experience.
  • 4+ years of experience in healthcare management or clinic operations, with a proven ability to influence metrics through data-driven strategies.
  • Understanding of clinical workflows and protocols.
  • Familiarity with HCC/RAF and HEDIS/Stars/Quality metrics preferred.
  • Ability to work effectively in a team, demonstrating sound judgment and problem-solving skills.
  • Excellent communication skills, both oral and written, suitable for diverse audiences.
  • Strong analytical skills to interpret data for performance improvement.
  • Proficiency in Microsoft Office 365 and population health management technology platforms.
  • Willingness to travel to provider offices as required.
  • Experience in Primary Care is strongly preferred.
