**Job Summary:**
As a Home Infusion Nurse Coordinator, you will be responsible for coordinating and providing direct infusion care to patients in their homes. You will work under the supervision of our experienced Regional Nurse Manager and will be an integral part of our clinical team. Your primary responsibilities will include assessing patient needs, developing individualized care plans, and educating patients and families on proper care techniques. You will also be responsible for documenting patient progress and collaborating with other healthcare professionals to ensure comprehensive care.

Key Responsibilities:
* Assess patient needs and develop individualized care plans
* Provide direct infusion care to patients in their homes

Requirements:
* 2+ years of experience in infusion therapies
* Current active and unrestricted registered nurse license
* Strong background in infusion therapies and excellent communication skills
* Ability to work independently and collaboratively with our clinical team
