POSITION OVERVIEW
The selected candidate will be tasked with implementing medical management and health education initiatives for clients involved in government health programs. Responsibilities will encompass the collection, analysis, and presentation of data for compliance reports.

ESSENTIAL QUALIFICATIONS
  • A minimum of 3 years of recent RN experience or one of the following:
    • Licensed Professional Counselor (LPC)
    • Licensed Master Social Worker (LMSW)
  • Valid IL or Compact RN License
  • 3+ years of experience in wellness or managed care
  • Familiarity with the health and wellness sector and employer trends
  • Experience in data analysis, particularly medical data
  • Proficiency in PC applications including Word, Excel, and PowerPoint, as well as database and web-based tools
PREFERRED QUALIFICATIONS
  • 3+ years of clinical experience
  • Experience in patient education
  • Background in Condition Management
  • Bilingual proficiency in English and Spanish
  • Experience in Transition of Care
  • Ability to manage complex or catastrophic cases
  • Certification in Case Management, Training, Project Management, or other nationally recognized healthcare credentials
