We foster health, wellness, and systemic change through outreach, care services, advocacy, training, prevention, education, and capacity building.

This role plays a vital part in our mission by serving as the key resource facilitator for individuals facing challenges with co-occurring social determinants of health, mental health, and substance use disorder.

Job Responsibilities:
  • Provide effective case management to ensure clients can set, monitor, and achieve goals identified to chart a path to stability and autonomy.
  • Collaborate with the care team to develop and implement an individualized health and wellness plan based on client-identified goals.
  • Engage referred clients, build trusting relationships, and provide individually tailored case management services.
  • Advocate for clients to gain access to a wide variety of community resources.

Essential Duties:

  • Develop and maintain client charts and case notes for assigned caseload according to program, contract, and state requirements in digital and paper filing systems.
  • Assist with program evaluation by facilitating timely completion of required documentation, assessments, and client encounters.
  • Perform other duties as needed or assigned.
Requirements:
  • Bachelor's degree in social work or a related field.
  • Peer recovery specialist certification or equivalent.
  • Ability to work productively, both independently and as part of a team.
  • Excellent interpersonal, written, and verbal communication skills.
Preferred Qualifications:
  • Master's degree in social work or a related field.
  • Certified Substance Abuse Counselor or equivalent.
  • Two years of experience working with populations living with homelessness, substance use disorder, and criminal justice involvement.
