- Communicate findings in your assessment to inform members' PCPs of potential gaps in care">

Benefits:
">We offer a competitive salary range of $40,000 - $60,000 per year, depending on experience, plus a $30,000 sign-on bonus for individuals who have not previously participated in this program. Additionally, we offer flexible provider schedules available between 20-40 hours per week. Required qualifications include an active NP or PA license or the ability to obtain one by the start date, active national NP or PA certification or the ability to obtain national certification in the state of assignment by the start date, and reliable transportation to complete home visit assessments.
