Job Summary:

We are seeking a skilled Senior Care Clinician to join our team at Optum Home and Community. As a key member of our interdisciplinary care team, you will provide high-quality patient care to individuals in assisted living and other community-based settings.

About the Role:

In this rewarding position, you will conduct comprehensive assessments, manage patients through acute medical situations, and ensure accurate documentation. You will also communicate effectively with patients, families, and the care team to develop and implement personalized care plans.

Key Responsibilities:

  • Conduct advanced illness and end-of-life discussions
  • Provide counseling and education to patients and caregivers
  • Facilitate agreement and implementation of patient care plans
  • Coordinate care across different levels and settings
  • Review orders and interventions for appropriateness and response to treatment

Requirements:

To be successful in this role, you must have a strong foundation in geriatric medicine and experience working with patients who require complex care. A Master's degree in Nursing or Doctor of Nursing Practice is required, along with active and unrestricted Nurse Practitioner licensure in the state where you reside. Board certification from the American Academy of Nurse Practitioners or American Nurses Credentialing Center is also necessary.

Benefits:
