Qualifications:
Applicants must fulfill educational prerequisites prior to application submission.
Bachelor's Degree in Health or Human Services with a minimum of 1 year of relevant experience, or an equivalent combination of education and experience in the fields of aging or disabilities; alternatively, a Registered Nurse with no less than 2 years of professional experience.
Responsibilities:
Conducts case management functions for individuals utilizing Participant Directed Services and integrated services; acts as a communication bridge between the participant and relevant agencies; performs associated tasks as necessary.
Guides participants in the creation and modification of their PDS care plan/support spending plan, adhering to person-centered planning methodologies and principles.
Facilitates training for participants and their hired staff on self-determination concepts.Oversees background verification processes for PDS contractors.
Keeps comprehensive records and documentation, including case notes.
Assists participants in acquiring goods and consumable medical supplies in accordance with their approved POC/SSP through a voucher system.
Coordinates with traditional service providers to ensure timely reassessments.
Offers technical support to participants in budget management, expenditure tracking, and contractor documentation maintenance.
Helps participants develop an emergency backup strategy, which may involve organizing emergency service provisions if required.
Requests authorization for additional services and funding as necessary.Assists participants in the hiring, training, scheduling, and termination of service providers.
Supports participants in finding service providers and negotiating service rates.
Processes contractor enrollment documentation and submits it to the financial management agency for approval.
Assists participants in transitioning to PDS services and coordinating between PDS, traditional waiver services, state plan services, and community resources.
Monitors participant adherence to the POC/SSP and service delivery to ensure compliance with the established guidelines and prior authorization limits.
