PDS Service Advisor/Case Manager

We are seeking a highly skilled and dedicated PDS Service Advisor/Case Manager to join our team at Lake Cumberland Area Development District. As a key member of our team, you will play a vital role in providing case management activities for individuals receiving participant-directed services and blended services.
Key Responsibilities- Provide case management activities for individuals receiving participant-directed services and blended services, serving as a liaison between the participant and DAIL and Medicaid.
- Assist DAIL with outreach to and enrollment of participants into PDS.
- Assist participants with the development and revision of the PDS plan of care/support spending plan utilizing a person-centered planning process and guiding principles.
- Assist with training of participants and their contracted employees on the principles of self-determination.
- Facilitate background checks of PDS contractors.
- Maintain all records and documents, including case notes.
- Assist participants with purchasing goods/consumable medical supplies based on the participant's approved POC/SSP utilizing a voucher system.
- Coordinate with traditional service providers to ensure reassessments are done timely.
- Provide technical assistance to participants with managing the budget, spending, and maintaining contractor records.
- Assist participants with the development of an emergency backup plan which may include arranging for the provision of emergency services if necessary.
- Request authorization of additional services and funding if needed.
- Assist participants in hiring, training, scheduling, and terminating service providers.
- Assist participants in locating service providers and negotiating rates.
- Process contractor enrollment forms and forward to the financial management agency and to DMS for approval.
- Assist participants with transitions to PDS services and coordination of PDS, traditional waiver services, state plan services, and community resources.
- Monitor participants' compliance with the POC/SSP and provision of services to ensure that service provision is within the scope of the POC/SSP and prior authorization limits.
- Perform all other duties as assigned.
We serve as regional planning agencies, forums, clearinghouses, technical centers, and as conveners for the region while administering numerous federal, state, and local programs. Unlike many other organizations structured along multi-jurisdictional lines, ADDs have both federal and state statutory authority.
