Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, infusion therapy, and other specialized services.
  • Conduct initial evaluations and regular reevaluations to assess patient needs and develop individualized plans of care.
  • Initiate and revise plans of care as needed, working closely with patients, families, and healthcare professionals.
  • Coordinate services with other healthcare professionals, including physical therapists, speech therapists, and medical social workers.
  • Prepare and maintain accurate and timely clinical and progress notes, summaries of care conferences, and other required documentation.
Requirements:
  • Graduate of an approved school of professional nursing and currently licensed in the state of Michigan.
  • One year of nursing experience, with a preference for experience in home healthcare, wound care, and infusion therapy.
  • Experience with Kinnser EMR and other electronic medical records systems is preferred.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
