Job Summary

Pillar Care Continuum is seeking a skilled Habilitation Specialist to support consumers in achieving their Individualized Habilitation Plan (IHP) goals. As a key member of our team, you will work closely with Senior Staff to develop and implement daily program activities that promote independence, dignity, and respect for all consumers.

Key Responsibilities:

  • Advocate for consumers and promote a culture of respect and dignity
  • Collaborate with Senior Staff to determine daily program activities and support consumer participation
  • Assist consumers with personal needs, including transferring, toileting, cleaning, grooming, and feeding
  • Collect and record data as directed, including completing daily DTR's within the Electronic Client Record
  • Participate in community activities with consumers as assigned by Senior Staff

Requirements:

  • High School diploma or equivalent
  • Valid Driver's License
  • 1-2 years of successful employment with a similar establishment or agency
  • Successful completion of criminal background history and fingerprint examination

Work Schedule: Monday - Friday, 8am - 4pm
